With an agreed upon set of expectations we are able to place our focus on providing excellent chiropractic care and helping you achieve better overall health.
To help establish a mutual understanding of the shared rights and responsibilities we have created the following document titled Performance Chiropractic Patient Bill of Rights. Additionally, you will find Performance Chiropractic Notice of Privacy Practices, which is our communication to you that is required by law (HIPAA) and intended to explain your rights and our responsibilities specific to how your protected health information (PHI) is securely obtained, maintained and disseminated (when necessary).
